	Covenant Sheet	Mail or Fax to ASP 2 weeks pre-trip along with Volunteer Trip Balance	Each member of your group acknowledges having read ASP Rules and Regs, 3s's, and ASP Safety Manual
	Payment Form	Due to ASP 2 weeks pre-trip	$85 per person
	Construction Information Form	E-mail or Fax to your center 2 weeks pre-trip	Summary of your group's skill level. Influences staff project selection for your group.
	Volunteer Statement and Registration Form	Due upon arrival at the center. Also, bring one set of copies for each vehicle.	Liability release and medical information form - must be notarized. Bring to the center.
	Weekly Family and Volunteer Work Crew Roster (form)	Due upon arrival at your center. Bring two copies	Necessary for purposes of locating a volunteer in case of an emergency. Bring to the center
